Ask most baseball parents how much their son threw last week and they will answer with a pitch count from his one start. That number is real, and it is also a fraction of the truth. Between bullpens, warm-up throws, long toss, infield reps, and the throws that never make it into any official log, a young player’s arm does far more work than the scoreboard records. As the summer season winds down and fall ball ramps up — one of the busiest throwing stretches of the year — that gap between counted and actual workload is where a lot of arms quietly get into trouble.
The number you track isn’t the number that matters
A 2024 review of youth and adolescent baseball workload in Current Physical Medicine and Rehabilitation Reports makes the point bluntly: pitch counts alone do not capture total throwing volume. They miss practice throws, warm-ups, and the between-inning work, all of which load the same elbow and shoulder. The review also flags a problem that any coach recognizes — perceived effort does not match measured effort. Players systematically underestimate how hard and how much they are throwing, which means the person best positioned to say “that’s enough” is often the least accurate judge.
This is not an argument against pitch counts. Guidelines like MLB’s Pitch Smart limits remain the sensible floor, and families should follow them. The point is that the pitch count is a floor, not a ceiling on the truth. A player who “only” threw 45 pitches in a game may have thrown another hundred that day that nobody wrote down.
The pitch count tells you what happened on the mound. It says nothing about the bullpen before it, the long toss after it, or the second game on Sunday.
Why fall is the pinch point
The danger window is not a single big outing. It is accumulation — a heavy stretch of throwing stacked on top of a body that is still growing. The same body of research points to peak growth years, when strength, power, and velocity climb at the same time the skeleton is still immature, as an especially vulnerable period. Add a calendar where summer showcases blur straight into fall ball with no real break, and you have volume rising exactly when the margin for error is thinnest.
The research community’s answer is not a magic number but a habit: watch the ratio between recent, acute workload and the longer chronic baseline a player is conditioned for. Spikes are the risk. A kid who threw lightly in July and then jumps into a full fall schedule in August is exactly the acute-to-chronic spike the literature warns about — even if every individual outing stayed under the pitch limit.
What a parent can actually do
Start by counting more than the game. For a week, keep a rough tally of all your son’s throwing — bullpens, long toss, warm-ups, the works — not to police it, but to see the real number. Most families are surprised, and that surprise is the point.
Then respect the transition. If summer just ended, the arm that carried it needs the ramp into fall to be gradual, not a cold jump into a packed weekend. Avoid stacking a fall-ball doubleheader on top of a lesson bullpen and a long-toss session in the same forty-eight hours. And build in genuine rest — days with no throwing at all — because chronic load without recovery is the pattern the research keeps pointing back to.
None of this requires a wearable sensor, though the review notes those tools are getting better at quantifying load (with the caveat that where you place the sensor changes what it reads). The National Strength and Conditioning Association’s long-term development guidance lands in the same place from the other direction: the athletes who get hurt are often the underprepared ones asked to do too much, too soon, without the conditioning base to absorb it.
The season your son plays is measured in pitch counts. The arm he keeps is measured in everything else. Counting only the first is how good intentions still end up in a doctor’s office in November.
